This text is based on material presented to medical students, residents and physician assistants in the classroom and clinical setting and in grand rounds presentations in departments of neurology and psychiatry at NYU, Cabrini, Woodhull, Manhattan State, and other local institutions. Valuable feedback from these settings has enabled the authors to hone their presentation to be of value to this wide compass of readership.
