The combination dual implant (preferably DHS+DFN/CCS+DFN) for treatment ipsilateral proximal and shaft femur fractures little better than single implant. They creates bio-mechanically stable construct allow early full weight bearing( partial weight bearing early in Long PFN but delay in full weight bearing because of delay union of shaft). Distal nail provide polar moment of inertia and proximal plates provide static moment of inertia at fracture site which was not hindering the union at shaft. Open reduction provide good exposure and easy for reduction at fracture site manly in unstable and communited fractures of femur.Combination of two implant construct have reduces the strain of single implant on any one of them which prevent expulsion or breakage of screw and implant. There was infection rate little higher in dual implant in compare to single implant which was reduces by maintaining intraoperative handling of soft tissue, maintaining sterility, antibiotic coverage and by reducing the time of surgery.It is therefore concluded that the result of combination dual implant is slightly better than single implant fixation.

Dr Deepak Rai is MS(Ortho) from AMU and also MNAMS and is currently working as Senior Resident in Trauma centre, IMS, BHU. Prof Ajit Singh is an alumnus of IMS, BHU and has a vast experience of about 15 years in managing such complex fractures.
