Boundary infostructures aim to enable pragmatic interoperability so the intended effect of a communication is the same as its actual effect. They are built from networks of boundary objects. These are objects that are flexible enough to be used in more than one context and to communicate between contexts. They include terminology systems, information architecture standards, and repositories. In this age of eHealth and chronic disease management, we need tools to mediate learning at the boundary between and among different communities of practice. Topic maps are used to represent the boundary infostructures and provide a common point of reference for clinicians, health informaticians, administrators, medical educators and patients. This work illustrates how a boundary infostructure was constructed and how it improved communication about patients with chronic disease.
