Our work aims to introduce a modified way to perform the transtibial technique ''TTT'' for the anterior cruciate ligament reconstruction. This technique would put the ACL graft in a more anatomic position giving improved clinical and radiological results compared to the traditional transtibial method. We challenged our approach to the anteromedial technique, the one with better results claims, but also has its risks and hazards. We believe that the current modified technique may solve the problem of vertical graft positioning, widely known for the TTT. It may also overcome the challenges of the anteromedial technique, namely the difficulty in obtaining a clear view of the lateral wall of the notch, the horizontal trajectory of wire drilling and reaming that increases the risk of articular cartilage damage and posterior cortical breakthrough. Also, the resultant acute pending of the inserted graft at its entry through the femoral tunnel will make the graft more jeopardised in the AAMPT.
