Paul Gilbert is Professor of Clinical Psychology at the Mental Health Research Unit at Kingsway Hospital, University of Derby and Derbyshire Mental Health Services Trust. Jeremy Miles is Lecturer in Biostatistics at the Department of Health Sciences, University of York.
