Meyer-University of Florence, Italy.
Mark Cook is Professor and Director of Neurology, St. Vincent's Hospital, Melbourne, Australia.
Samden Lhatoo is a Professor of Neurology at the Department of Neurology, Case Western Reserve University Hospital, Cleveland, USA.
